Congratulations on the birth of your child. The Baby Bundle is a gift from the NSW Government to help you with the first crucial months of your baby’s life.

This survey is intended for families who have received the Baby Bundle.

If your baby is not yet born, please note that parents and caregivers will receive a Baby Bundle when they are discharged from a public or private NSW hospital.

If your baby was born interstate or overseas and you are a NSW resident, we can arrange a Baby Bundle to be sent to you.

To be eligible for a Baby Bundle, your baby must be born after 1 January 2019 and you must live in NSW. If you have not received a Baby Bundle but believe you are eligible, please contact MOH-BabyBundles@health.nsw.gov.au and let us know:

• your address

• your baby's date of birth

• where your baby was born.
