Aboriginal Nursing and Midwifery Undergraduate Scholarship recipients

This submission form is intended for current recipients of the NSW Health Aboriginal Nursing and Midwifery Undergraduate Scholarship program.

Scholarship payments are actioned by the completion of this form at the commencement of each semester or trimester. Please ensure you submit evidence of your enrolment and your previous semester/trimester results to prevent delays in processing.

Payment will be for one semester or trimester at a time, this form will need to be submitted at the beginning of each semester or trimester. For students studying double weighted subjects, these subjects will be worth 2 units ($2,000).

If you are not currently in receipt of this scholarship program your submission will not be processed.
